• Form 1099-DIV, Dividends and Payments, is sent to investors who receive payments from any type of investment during the calendar year.

  • Banks, investment companies and other financial institutions must provide taxpayers with the form by January 31 of each year.
  • Taxpayers enter the information from each Form 1099-DIV either on the Schedule B form or directly on the Form 1040.
  • Taxpayers are only sent the form if the dividend and/or payout they receive is more than $10.
  • Financial institutions must submit the form to both the taxpayer and the IRS.
